8. Is the penalty premium recalculated every year based on that year's national average Part D plan premium?

Yes.  The penalty is recalculated every year based, in part, on the national average of Part D plan premiums.  Information about the exact calcualtion is available from Section 423.286 of the MMA Regulations.
